As a result of our rebrand to Resolution Life, we have updated the PDS for Elevate Insurance.

  • New PDS: Elevate Insurance PDS and plan document, 19 December 2022

  • Old PDS: AMP Elevate insurance PDS and plan document, 1 October 2021 

Medical definitions review

We used this opportunity to review our medical definitions to ensure they remain current with medical practices and measures.

The updates were predominantly focused on applying the outcomes from the work done for Unfair Contract Terms (UCT). This included removing the terms ‘approved by us’, ‘in our opinion’, and ‘acceptable to us’.

We also replaced the reference to a specific medical specialist with ‘appropriate specialist medical practitioner’ for several definitions. Providing a more general description of the specialist required will allow customers who do not have access to the required specialist due to location, access to a medical specialist who could confirm diagnosis. This will also support claim assessors in processing claims efficiently.

These changes have been made with no change to premium rates and will be passed back to all Elevate Insurance customers.

New business post 19 December 2022

Any new policy issued from 19 December 2022 will be issued under the new PDS, even if the application was submitted on an application form from the old PDS. There will be no requirement for clients to sign an updated application form and these clients will be advised that they are able to access the new PDS on our public website.

Stand-alone application form and personal statement for Elevate Insurance

The application form and personal statement have been removed from the PDS. These forms can be downloaded from our public website. The Elevate Insurance declarations, consent and signatures form can be used if you are completing your client’s application via Elevate Online.
