If your policy has lapsed, or in other words, cancelled due to non-payment of premiums you may be able to reinstate your policy within the first 90 days by completing the online application form below:

For unsuccessful digital reinstatements or after 90 days from lapse date you may apply to reinstate your insurance by completing one of the following applications. Please choose the reinstatement form appropriate for your insurance plan and circumstance, complete in full and email to insurance@resolutionlife.com.au
