Complete a withdrawal form

To initiate a withdrawal, you must complete and submit a withdrawal form.

You can find the form relevant to your product and your specific situation in our Find a Form page.

Please download this form, ensure that you fill out all sections and return it via post along with a copy of your policy document, any memorandums of alteration or additional insurance, and a certified original of some form of identification with your (and any other policy owner’s) signature.

Lost your policy document?

If you’ve misplaced your policy document, you can request a replacement by completing our Application for replacement plan form. 

Provide identification

To meet our legal obligations and ensure precise disbursement, each policy owner must complete the Know Your Customer (KYC) Form.

You can find the form that suits your product and your specific situation in our Find a Form page.

Alongside your KYC form, you will need to submit certified copies of your identification documents.

Accepted forms of identification

The following documents are generally accepted as adequate identification. You will be required to provide documents as outlined below:

One document listed in Option A 

OR 

One document from Option B 

PLUS

One document from Option C

Certifying your identification 

Here is what you need to do.

1. Photocopy your identification documents

Begin by making photocopies of your identification documents.

2. Get the copies of your identification documents certified

Once you have photocopied the necessary identification documents, the next step is to get these photocopies certified. Only specific individuals such as those listed below are authorised to certify these documents. You can find a complete list of eligible certifiers at Identification - Resolution Life Identification

• Police officer or sheriff
• Justice of the peace or notary public
• Legal practitioner, magistrate, judge, or registrar/deputy registrar of a court
• Pharmacist, physiotherapist, or veterinary surgeon
• Optometrist, dentist, medical practitioner, or nurse
• Member of the Institute of Chartered Accountants in Australia, the Australian Society of Certified Practicing
• Accountants or the National Institute of Accountants

Finally, send us your withdrawal form with certified copies of your identification documents. Please be aware that we don’t accept facsimiles or copies of the certified documents. Post your forms and certified documents to the address at the bottom of your withdrawal application form.
